All physical forms and other parent sign-offs will be uploaded and completed using the Rank One app. You can find information and instructions by navigating through this website.
You will need to print a copy of the KSHSAA physical form and take it with you to your child's exam. The physical must be completed in its entirety before your child is eligible to participate this fall. This physical is valid for one full school year. Once your physical is completely filled out, you will take pictures of all four signature pages and upload them to the Rank One app using a cell phone. Please do not bring the forms to me! I cannot upload on your behalf! You will complete all other parent sign-offs on the Rank One app as well.
